New Patients
To prepare for your first office visit, please take care to:
- download and complete all of our New Patient forms (NP1 - NP4) below prior to your first appointment
- be sure that all completed New Patient forms are received by our office at least one day prior to your scheduled initial appointment
- wear comfortable clothing
- allow one and one half hours for your first appointment and one hour for subsequent visits
- arrive at least 10 minutes early
- bring your insurance card & all recent lab results with you
- contact us with at least 24 hours notice to change or cancel your appointment
- remember that payment is due at the time services are rendered
- we accept Visa & MasterCard, Health Savings account Visa or MasterCard, personal and Health Savings account checks, and cash (if you do not have exact change a credit will be left on your account)
Provided your policy covers your acupuncture treatments, we currently bill the following insurance companies:
- Aetna
- United Health Care
- Cigna
- Blue Shield PPO
We currently do not bill Anthem Blue Cross, but instead we will provide you with an itemized invoice for you to submit a claim directly to them.
